Reporting Requirements
- Completed FY 2025 ADH Cost Report Template
The ADH cost report is submitted to CHIA online through CHIA Submissions. If you are a new provider or new user and do not have log in credentials, you must register with CHIA as soon as possible (see below).
- FY 2025 Financial Statements
Financial Statements must be emailed as a PDF to data@chiamass.gov.
Reminders
- Read the Cost Report Instructions and the CHIA Submissions User Guide found in the “Data Submission Resources” section before beginning work on the cost report.
- CHIA Submissions works best in Google Chrome.
- Data must be keyed in. Do not insert formulas or references to other spreadsheets because this may prevent the data from calculating accurately.
- All numbers must be whole numbers without decimals except for hours and FTEs reported in Schedule B (Program Information) and Schedule D (Staffing Information).
- In Schedule D, if you have no information to report for a given staff type, leave the cell blank.
- The “Certification by Provider” section in the blue cells in Tab 9 must be filled in to save successfully.
- All submissions must be saved using the “Save Submission File” button on Tab 9, Accuracy of Report. Submissions that are not saved by using this button cannot be read by CHIA’s system and will not successfully upload.
Registration Instructions
New users must register to submit data via CHIA Submissions.
Complete User Agreement
Complete and sign the CHIA Data Reporter User Agreement (DRUA) after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
Submit User Agreement
Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov. You will be notified when your account is ready to use.
