Reporting Requirements
TNS agencies are required to submit the following no later than June 1, 2026:
- Completed FY 2025 Cost Report (Excel)
- Download the cost report template from the Report Resources box on the right side of this page.
- Save the file using the following naming convention:
AgencyName_DPH ID_FY25_TNS Cost Report. - Email the completed Excel template to tnscostreports@chiamass.gov.
- FY 2025 Financial Statements (PDF format)
- These documents must be submitted in PDF format.
- Save the file using the following naming convention:
AgencyName_DPH ID_FY25_TNS Financial Statements. - Email these douments to tnscostreports@chiamass.gov.
The email subject line should read: TNS FY 2025 Cost Report from [Agency Name and DPH ID]

Read the Cost Report Instructions and the CHIA Submissions User Guide linked above before beginning work on the cost report.
Reconciliation of any differences between the financial statements and cost report is now incorporated into the cost report.
