Reporting Requirements
GAFC providers must send a properly completed GAFC Cost Report. Cost reports containing any errors listed in the “Unacceptable Filings” section of the cost report instructions will not be considered submitted until all errors are fixed.
Save your cost reports and certification statements electronically, using the file naming convention specified in the cost report instructions. Attach the following to an email and send to CHIA at data@chiamass.gov:
- Completed Excel workbook cost report named in the format “AgencyName_GAFCCR25.xlsx” where AgencyName is the name of your agency
- PDF version of signed Statement of Certification named in the format “AgencyName_GAFCCR25.pdf” where AgencyName is the name of your agency
- Copies of PDF financial statements named in the format “AgencyName_GAFCFS25.pdf” where AgencyName is the name of your agency
In the subject line of the email, enter “GAFC FY2025 Cost Report from [your agency name]”.
Registration Instructions
New users must register to submit data via CHIA Submissions.
Complete User Agreement
Complete and sign the CHIA Data Reporter User Agreement (DRUA) after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
Submit User Agreement
Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov. You will be notified when your account is ready to use.

Provider Financials/Cost Reports
