Reporting Requirements
CSN agencies are required to submit the following to csncostreports@chiamass.gov no later than June 29, 2026:
- Completed FY 2025 CSN Cost Report (Excel)
- Download the cost report template from the Report Resources box on the right side of this page.
- Save the file using the following naming convention:
AgencyName_MassHealthID_FY25_CSN Cost Report - Email the completed CSN Cost Report Template to csncostreports@chiamass.gov
- FY 2025 Financial Statements (PDF)
- These documents must be submitted as PDFs.
- Save the file using the following naming convention:
AgencyName_MassHealthID_FY25_CSN Financial Statements - Email the financial statements to csncostreports@chiamass.gov
The email subject line should read: CSN FY 2025 Cost Report from [Agency Name and MassHealth ID].
Additional Information
- Please read the Cost Report Instructions and the CHIA Submissions User Guide found under “Report Resources” on the top right of this page PRIOR to beginning work on the cost report.
- The CSN cost report will now be completed in Excel template format and emailed to csncostreports@chiamass.gov.
- Reconciliation of any differences between the financial statements and cost report is now incorporated into the cost report.
Registration Instructions
New users must register to submit data via CHIA Submissions.
Complete User Agreement
Complete and sign the CHIA Data Reporter User Agreement (DRUA) after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
Submit User Agreement
Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov. You will be notified when your account is ready to use.

Provider Financials/Cost Reports
Provider financial and cost reports are submitted from acute hospitals, affiliated physician groups, hospital health systems, hospitals, skilled nursing facilities, resident care facilities, and other providers.
