Reporting Requirements
Community health centers must file the following reports, as applicable:
Community Health Center Cost Report
Read the Cost Report Instructions before you begin working on the cost report. Agencies must file the cost report based on the provider’s fiscal year end. The reporting period should be consistent with financial statements. The cost report needs to be completed on the basis of total program costs. The cost report is submitted via CHIA Submissions (see below).
Financial Statements
Agencies must submit copies of financial statements and other external documentation supporting the accuracy of the data reported on the cost report. See the Cost Report Instructions for a list of acceptable documentation. Email financial statement documents to data@chiamass.gov.
Label financial statement files with the CHC name and document type, for example: AgencyName_CHCFS_FY25.pdf
Registration Instructions
New users must register to submit data via CHIA Submissions.
Complete User Agreement
Complete and sign the CHIA Data Reporter User Agreement (DRUA) after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
Submit User Agreement
Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov. You will be notified when your account is ready to use.
