New users must register to submit data via CHIA Submissions.
Complete User Agreement
Complete and sign the CHIA Data Reporter User Agreement (DRUA) after reviewing the DRUA instructions. Note that each individual user for an organization must complete the form.
Submit User Agreement
Submit the completed Data Reporter User Agreement to DL-Data-Submitter-HelpDesk@chiamass.gov. You will be notified when your account is ready to use.

Additional Information
CHIA will continue to collect sexual orientation/gender identity (SOGI) patient data reported in the fields below but will not be sharing this information with MassHealth for individuals younger than 19 years of age as of December 31 of the measurement year. This change does not affect the EHRD File Submission Guide.
