Information for Data Submitters: Ambulance and Wheelchair Van Cost Reports

2021 Report Resources


Submission Deadline

  • The 2021 Ambulance and Chair Car Service Cost Reports were due by Wednesday, October 12, 2022.


The Center for Health Information and Analysis (CHIA) is now collecting cost reports for the fiscal year (FY) 2021 from ambulance and  wheelchair van providers. CHIA collects these reports pursuant to regulation 957 CMR 6.00. The reports are used by CHIA and other public entities for monitoring costs and for health care payment policy development.


What's New for 2021

The FY2021 Ambulance and Wheelchair Van Cost Report has been redesigned as an Excel template to be submitted through CHIA’s online submission portal, CHIA Submissions. The template closely follows what was previously submitted in INET. We expect that this redesign leads to easier usability for providers. Please closely read the Cost Report Instructions and CHIA Submissions Guide for detailed instructions on how to complete and submit the new and improved cost report.

All non-municipal ambulance and wheelchair van providers are required to fill out the cost report. Providers that earned more than $100,000 from MassHealth for services provided in FY2021 are required to fill out the full cost report. Providers that earned less than $100,000 or no dollars from MassHealth are only required to fill out a partial report. Please see the Cost Report Instructions for more information.

Ambulance and wheelchair van providers are required to submit the following:

  • FY21 Cost Report (Excel Template)

  • FY21 Financial Statements


The reports were due no later than Wednesday, October 12, 2022.

CHIA will only accept cost reports via CHIA Submissions. Financial statements should be emailed directly to CHIA in PDF format at Links to instruction documents are posted on this webpage. Please read all instructions carefully.

Access CHIA’s online submission platform at For users who were previously registered with INET, the same login credentials can be used. Please see the CHIA Submissions User Guide for how to register if you are a new user.

  1. Name of your agency with MassHealth ID #
  2. Name of the cost report you are asking about
  3. Cost report reporting year
