Overview

CHIA is now collecting cost reports for fiscal year (FY) 2025 from Group Adult Foster Care (GAFC) providers. CHIA collects these reports pursuant to regulation 957 CMR 6.00. The reports are used by CHIA and other public entities for monitoring costs and for health care payment policy development. Dual providers of both GAFC and Adult Foster Care (AFC) services must file separate cost reports.

Reporting Requirements

GAFC agencies are required to submit the following no later than May 18, 2026:

• Completed FY 2025 GAFC Cost Report
• FY 2025 Financial Statements
• Statement of Certification

Due Date

Agencies must submit reports and all supplemental items directly to CHIA at data@chiamass.gov no later than May 18, 2026. CHIA will accept cost reports in .xlsx format only. Supporting documents such as the Financial Statements and Statement of Certification should be in PDF format.
