Information for Data Submitters: Community Health Center Cost Reports

Overview

CHIA has issued the Community Health Centers’ (CHC) Cost report for Fiscal Year (FY2025). The reports are used by CHIA and other public entities to monitor costs and to develop health care payment policy. CHCs must submit their data in accordance with 957 CMR 6.00 and the filing requirements set out in the cost report filing instructions, which can be found in the blue box on the right of this page. Please reference the instructions document for information on completing the CHC Cost Report.

Reporting Requirements

CHCs are required to submit the following, due no later than July 7, 2026.

• FY 2025 Community Health Center Cost Report Template

  • If you cannot find your provider name in the CHC Cost Report template or you are a new filer with CHIA, you must fill out, scan, and email a CHIA Data Reporter User Agreement to CostReports.Pricing@chiamass.gov before you begin working on the cost report and attempt to submit a completed cost report.

• Name of the agency with MassHealth ID#
